Dashboard Request Form - Case Management Software

Thank you for your interest in Trialworks Dashboards! Before you request/pay for a custom dashboard, be sure to check out our Trialworks Dashboard Library. All the dashboards listed in this document are available free of charge. If you still need a new custom dashboard created, please complete the following survey. This form must be submitted for all requests relating to the creation of new dashboards and/or modifying existing dashboards.

Pricing: Assembly Software staff will review your request in the order received. We will contact you if additional information or data is required. Once the request has been evaluated, we will provide you with the estimated cost for developing your request. An invoice will be sent in advance, and payment must be received before we will begin report writing services. Please be as detailed and accurate as possible when filling out your request.

If you are uncertain of the proper name of the field, provide the Tab name where the field appears and the name of the label in front of the field. Click the + to add as many rows as you need.

For example, if the report is to perform calculations or only present data within a specific timeframe, describe those conditions. For example: "The dashboard should show the percentage of cases that are of a specific Case Liability Type" or "The dashboard should show the number of cases opened in a specific time frame that were generated by a specific marketing campaign" or "The dashboard should show the total amount of fees generated by referral attorney within a specific time frame."
